Proposal Procedures

Organizations invited to submit full proposals should send materials via email to submissions@kapfam.com. Applicants will receive an email to confirm receipt of the full proposal.

Please be sure that your proposal includes all the appropriate materials as outlined below. When possible, please send proposals in PDF format – one document with all attachments and supporting materials is preferred.

Full proposals should include the following:

Cover sheet or Letter

Include primary contact information, the amount requested and for what purpose, and the applicable program area.

Proposal Narrative

Should be no more than 6 pages and include a brief description of the organization, its history, the need for services provided, target population, and client demographics. The narrative should:

  1. include the organization’s (or program) goals, how those goals are measured, and any results to date;
  2. provide an overview of programs, how programs have changed or progressed, and plans for the future; and
  3. describe recent organizational and programmatic challenges and accomplishments.

Budgets

Income and expense budget for the year funding is requested and for the prior fiscal year. If requesting program-specific funds, please include a program budget for the current and prior years.

Funding Commitments

A list of grant commitments over $5,000 for the year funding is requested and for the prior fiscal year. If the organization has not received grants over $5,000, please list the organization’s top funding sources.

Potential Support

A list of all pending and potential grants over $5,000 for the year funding is requested.

Audit

One copy of the most recent external audit. If your organization is not required by law to obtain an external audit, please submit a board-approved report of the previous fiscal year’s actual income and expenses.

List of Board of Directors

A list of current board members and their aggregate financial contributions to the organization (example: 90% of our twelve board members contributed $$$ in 2010). If the board does not provide financial support, please explain their contributions of time and talent to the organization.

Tax Exemption Letter

A copy of a valid IRS 501(c)3 federal tax exemption letter.

IRS Form 990

The first section of the most recent IRS Form 990 (from the first page to the signature page, generally about 8 pages).
